You can look this up online at https://hcahealthcare.com/about/privacy-update.dot They have a list of affected states and the medical facilities from which data was stolen. Do not provide this phone number with your personal information and be wary of calls from medical facilities request exorbitant amounts of information from you. Instead, call the facility directly and request a mailed copy of an invoice for any balance you have due.
