It is legit. It is from United Health Care to schedule your annual physical exam. They will ask first and last name, date of birth, and address. They will then help you schedule an appointment with your listed PCP.

It’s a scam. United healthcare do call but they will call as a friendly reminder to schedule your annual appt. They don’t schedule the appts therefore if they were to ask us to verify any information it tells you it’s a scam.

Not a scam. If you call the number back, you'd know its united healthcare trying to schedule your annual physical because you haven't had one yet this year. They ask for name and DOB for HIPAA compliance, but no info beyond that.
