The CDC is conducting a post COVID-19 survey asking questions like will the people living in your house get COVID boosters, RSV vaccine, Flu shots, etc.; if you have health insurance; where do you go for vaccines, your awareness of no-cost vaccines, have you ever felt discriminated against at a clinic due to requesting a vaccine... They ask non-personally identifying questions, and at any time you can choose not to answer a particular question. None of the information can violate HIPAA because no names are provided, no addresses are given, and the phone number is not recorded. They ask for a zip code so they know they are getting a cross-section of the country. No harm to participate with this random survey.

Call from (404) 806-8411 claiming to be the CDC asking you to call (877) 220-4805 (a contractor) to complete an "immunization survey". Scammers use this tactic often. You don have to call back, you don't have to call the number and you don't have to complete the survey. Never give out any personal financial or social security information over the phone.
